- Observed unlabeled container of chili being stored in walk in cooler. Manager voluntarily discarded chili. Ensure all foods are clearly labeled with date of open or preparation and then discarded after 7 days. First date of opening counts as Day 1.
- Observed uncovered tray of bread being cooled on cart. Manager put a metal tray above bread, covering it. Ensure all foods are covered during storage.
- Observed soda syrup lines touching the floor. Ensure all foods, including soda lines, are at least 6 inches off the floor. Manager adjusted soda lines, so they are no longer on floor.
- Observed soiled plastic containers being used to store clean utensils on back storage shelf. Ensure all food contact surfaces are routinely washed, rinsed, and sanitized.
- Observed black debris on drip tray of ice machine. Routinely clean inside of ice machine to eliminate black buildup.
- No hand towels in men's restroom. Re-stock hand towels.

- Food contact surfaces must be clean. Observed the following visibly soiled: storage shelves, containers storing clean utensils, handles on walk-ins, prep tables, and the surface underneath the soda machine.

- Walls and ceiling shall be smooth and easily cleanable. Observed openings around pipes above water heater and CO2 tanks, missing ceiling tiles above water heater, and broken light switch covers. Repair or replace.
- Walls and ceilings must be clean. Observed visibly soiled walls throughout food prep area. Clean.
- Lights must be properly shielded. Observed broken light shielding above CO2 tanks. Repair or replace.
- Only necessary items shall be stored on the premises. Observed cardboard being stored for personal use in food prep area and un-used hose stored under storage shelves. Provide storage out of food area, or remove through other means.

- Found tape along the bottom of the mobile bread rack on the front line. Tape is not a smooth and easily cleanable surface. Remove tape.
- Found food debris build-up on the door seal gasket to the reach in prep cooler on the front line. Found food debris build-up inside the dough cart stored in the walk-in cooler. Wash, rinse and sanitize surfaces.
- Dumpster lids not closed. Close lids when not in use.
- Found soda syrup on the floor under the soda rack in the back. Clean floor.

- The microwave near the drivethru has a rack attached to the top of the microwave hanging down inside. Repair or replace microwave.
- The racks in the walk in freezer have paint chipping. Repaint and reseal the racks.
- There is dark build up in the ice machine drip tray. Clean the ice machine.
- All non-food contact surfaces need to be washed, rinsed, and sanitized. These surfaces include but are not limited to the soda syrup racks, cooler/freezer handles, behind the nozzles on the soda dispenser, the microwaves faces, and cooler/freezer door seal gaskets.
- Found clean knives stored in racks that were dirty. Baking pans were stored with the food contact surface presented. Store dishes in clean containers. Store pans inverted.
- Found plastic lids for sandwich platters stored with food contact surface presented. Store lids and all single service items inverted.
- Found cracked floor tiles in the kitchen/prep area. Replace or repair the floor tiles.
- Build up on the floors especially under the 3-compartment sink and the ice machine. Clean the floors.
- Found build up on the walls behind the 3-compartment sink and near equipment. The ceiling in the walk in cooler near drivethru has build up on it. There is a lot of dust build up on the floor/wall juncture in the women's bathroom. Clean the walls and ceilings.
- The air vent in the ceiling in the dish/prep area is dirty. Clean the vent.
- There is no light shield in the women's restroom. The light shield in the men's room is cracked and askew from the grid.
