    Observation: There are ready-to-eat foods stored under raw shell eggs in the prep cooler and in the commercial refrigerator.
    Correction: Reorganize storage so that ready-to-eat foods are stored above raw animal food and raw vegetables. Discard ready-to-eat foods that show evidence of contamination.
  • WAREWASHING - SANITIZING SOLUTION - TESTING DEVICES
    Observation: A chlorine test kit is not available for checking sanitizer concentrations. Strips were unable to be located today.
    Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions.
  • CERTIFIED FOOD MANAGER – REQUIREMENT
    Observation: John Grabrick has a ServSafe certificate, unable to locate in the WI Certified Food Manager database.
    Correction: Provide a Wisconsin Certified Food Manager (submit copy of ServSafe with DHS CFM application and fees to DHS) and post the state certificate in the food establishment within 90 days.
  • EQUIPMENT AND UTENSILS CONFORM TO ANSI STANDARDS
    Observation: Deep fryers are not ANSI certified.
    Correction: Unapproved equipment shall be removed from food service or provide documentation that the deep fryers meet ANSI standards.
  • VENTILATION HOOD SYSTEM - ADEQUACY
    Observation: The grill, deep fryers and paninni grill are not adequately covered by a ventilation hood system.
    Correction: Provide additional ventilation hoods or devices to prevent grease and condensation from collecting on walls and ceilings. Contact the department of safety and proferssional services for requirments on plan review and installation.

    Correction: Provide thermometers that are easily accessible and capable of taking temperatures in food. I provided a dial stem thermometer today. Obtain a thin probe thermometer capable of taking accurate temperatures in thinner food items.

    Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions. I provided 1 vial of chlorine test strips today.
  • TOXIC SUBSTANCES - SANITIZER CRITERIA
    Observation: Lemon scented bleach used for sanitzing.
    Correction: Use unscented bleach or other approved sanitizer.
  • CERTIFIED FOOD MANAGER – REQUIREMENT
    Observation: There is no Wisconsin Certified Food Manager for this establishment.
    Correction: Provide a Wisconsin Certified Food Manager and post the state certificate in the food establishment within 90 days. Correct By: 01-Jul-2014
  • THERMOMETERS - FUNCTIONALITY - INTEGRAL OR PERMANENTLY AFFIXED MEASURING DEVICE
    Observation: Walkin cooler does not have a thermometer and is being used to store open juices, cut fruits, etc for drinks.
    Correction: Equipment used for hot and cold holding shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device. Provide in a conspicuous location to monitor temperature is 41F or below.

    Observation: No commercial refrigeration available for food storage.
    Correction: Obtain commercial refrigeration for food storage.
  • TOILET FACILITIES - RECEPTACLES - COVERED
    Observation: Female bathroom is not provided with a covered waste receptacle.
    Correction: Provide a covered waste receptacle in female bathroom for use with sanitary napkins.
  • HANDWASHING SIGNAGE
    Observation: No handwashing signage provided at handwashing sink behind the bar.
    Correction: Provide handwashing signage at all handwashing sinks used by food employees. Provided a sign today.
