Observation: There is no detectable level of sanitizer in the warewashing machine.
Correction: Provide training to employees on proper use and concentration of sanitizer or repair or adjust warewashing equipment to provide proper sanitizer concentration. Operator adjusted machine and showed 50 ppm on utensils.

Observation: An air break exists between the sewage system and a drain from the ice machine. There is no vertical separation between the drain pipe and receiving pipe.
Correction: Design the sewage system in a manner that would preclude a direct connection between the sewage system and the drain from which food is placed. Provide an air GAP on the drain from the ice machine.

Observation: Food establishment is operating beyond their current food permit category assignment. Assessed complexity factors include raw animal food, seating over 50, presence of a self-serve salad bar, cooling, and food prep that includes slicing/dicing/boiling. Operator indicated only outside caterers provide food for banquet hall, and no food is provided by the on-site restaurant for the banquet hall.
Correction: Reduce current operation to meet current license restrictions or modify your current license and pay applicable fees to the department. All food code requirements for the new risk category shall be met before moving from one category to another.
