  • Critical: COMPLIANCE WITH FOOD LAW - UNAPPROVED SOURCE
    Observation: Frozen pizzas from Great Scott's Pizza in Mount Horeb are not approved. Inspector verified with Public Health Madison and Dane County and Dept. of Ag that Great Scott's Pizza is licensed as a restaurant and does not have a wholesaling or meat license. They are not allowed to wholesale.
    Correction: Discard foods from unapproved source, do not use the pizzas from the unapproved source. Operator will take the pizzas home. Obtain food items from approved sources only. Correct By: 17-Sep-2015
  • Critical: RTE/PHF-TCS PROPER DATE MARKING (corrected on site)
    Observation: Cut lettuce in the homestyle cooler is not date marked.
    Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days. Lettuce was datemarked and moved to a different cooler. Correct By: 17-Sep-2015
  • Critical: SEWAGE - BACKFLOW PREVENTION
    Observation: A direct connection exists between the sewage system and a drain from the ice machine and the ice bin.
    Correction: Design the sewage system in a manner that would preclude a direct connection between the sewage system and the drain from which food is placed. Remove the direct connection by adding an air gap between the drain from the ice machine and the ice bin. Correct By: 01-Oct-2015
  • VARIANCE OBTAINED FOR SPECIAL PROCESSES
    Observation: There is no documentation of an approved variance for reduced oxygen packaging of salami and pepperoni.
    Correction: Submit a variance application to the regulatory authority for submission to the State for review. If approved, a copy of the approved variance shall be kept at the establishment and be subject to review by the inspector upon request. Discontinue current process of vacuum packaging anything until a variance can be obtained from the State. Discard salami that has been vacuum packaged. Operator will take the salami home.   • WAREWASHING - SANITIZING SOLUTION - TESTING DEVICES
    Observation: A chlorine test kit is not available for checking sanitizer concentrations of the sanitizer used in the 4 compartment sink. Currently facility has a quaternary ammonia test kit, however, the sanitizer is a chlorine based sanitizer and the test strips do not work.
    Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions. Correct By: 17-Oct-2015

    Observation: There is no service sink provided in food establishment.
    Correction: Provide at least 1 service sink or curbed cleaning facility. Correct By: 17-Oct-2015

    Observation: No handwashing signage provided at bar handwashing sink.

  • CATEGORY ASSIGNMENT
    Observation: Food establishment is operating beyond their current food permit category assignment. Inspector noted raw bacon and a cut tomato and a cut head of lettuce in the establishment.
    Correction: Reduce current operation to meet current license restrictions or modify your current license and pay applicable fees to the department. All food code requirements for the new risk category shall be met before moving from one category to another. Operator has decided to cease operations to fit under the simple complexity license. Cut tomato and cut head of lettuce will be discarded. Raw bacon will be taken home with operator and will not be used. Operator will purchase precooked bacon if used. Correct By: 17-Sep-2015

    Correction: Provide at least 1 service sink or curbed cleaning facility.
  • CLEANING, FREQUENCY AND RESTRICTIONS
    Observation: The walk-in cooler upstairs has black mold spots under the shelves and cooling unit.
    Correction: Clean the physical facilities at a frequency necessary to keep them clean. Correct By: 30-Sep-2014
