Critical: PHF/TCS, COLD HOLDING (corrected on site) Observation: Raw chicken, beef and cut vegetables are cold held in the walk-in cooler for unknown period of time at 44-45°F. Affected items discarded. Thawing fish was at 41 F so was moved to functional cooler.
Correction: Maintain cold potentially hazardous foods at or below 41°F Assure adjustment and/or repair and monitor temp using additional thermometer
Critical: PLUMBING - BACKFLOW PREVENTION DEVICE - WHEN REQUIRED Observation: Plumbing system not provided with an air gap or backflow prevention at prep sink. There is one there but it is wrapped in plastic.
Correction: Provide air gap or backflow prevention device. Plumbling systems shall be provided with backflow prevention at each point of use in the food establishment. Wider funnel may be needed at receptacle side. Correct By: 12-Nov-2015

THERMOMETERS - AMBIENT AIR AND WATER - ACCURACY Observation: Thermometer in walk-in cooler measuring ambient air is not accurate. Dial at door reads 40 F though foods are at 44 F.

Correction: Store all foods that are removed from their original packages in containers labeled with the common name of the food or ingredient.
THERMOMETERS - FUNCTIONALITY - EASILY READABLE Observation: Thermometer in reach-in cooler in back (M3) is not easily readable.
Correction: Provide thermomters that are easy to read.
