Observation: Raw chicken, beef and cut vegetables are cold held in the walk-in cooler for unknown period of time at 44-45°F. Affected items discarded. Thawing fish was at 41 F so was moved to functional cooler.
Correction: Maintain cold potentially hazardous foods at or below 41°F Assure adjustment and/or repair and monitor temp using additional thermometer
- Critical: PLUMBING - BACKFLOW PREVENTION DEVICE - WHEN REQUIRED
Observation: Plumbing system not provided with an air gap or backflow prevention at prep sink. There is one there but it is wrapped in plastic.
Correction: Provide air gap or backflow prevention device. Plumbling systems shall be provided with backflow prevention at each point of use in the food establishment. Wider funnel may be needed at receptacle side. Correct By: 12-Nov-2015
- DEMONSTRATION OF KNOWLEDGE
Observation: The Person in Charge (PIC) is unable to demonstrate knowledge of cold holding of potenially hazardous foods
Correction: The person in charge shall obtain training or training materials in the areas of cold holding so they are able to demonstrate knowledge and train employees on proper food safety practices.
- THERMOMETERS - AMBIENT AIR AND WATER - ACCURACY
Observation: Thermometer in walk-in cooler measuring ambient air is not accurate. Dial at door reads 40 F though foods are at 44 F.
Correction: Calibrate or replace thermometer. Correct By: 12-Nov-2015
- LABELING - FOOD STORAGE CONTAINERS LABELED WITH COMMON NAME
Observation: Raw chicken is stored in unlabeled container.
Correction: Store all foods that are removed from their original packages in containers labeled with the common name of the food or ingredient.
- THERMOMETERS - FUNCTIONALITY - EASILY READABLE
Observation: Thermometer in reach-in cooler in back (M3) is not easily readable.
Correction: Provide thermomters that are easy to read.
- UTENSILS, EQUIPMENT AND LINENS - STORING PROHIBITIONS - CLEAN DRY LOCATION
Observation: Bowls stored improperly.
Correction: Clean equipment and utensils, laundered linens, single-use and single service articles shall be stored in a clean dry location, not exposed to contamination and off the floor.

Observation: Take-out containers not stored in original container and not properly protected
Correction: Store single-use articles in their original container or use other effective means to prevent contamination.
