Correction: Provide a Wisconsin Certified Food Manager and post the state certificate in the food establishment within 6 months of opening.
EQUIPMENT AND UTENSILS CONFORM TO ANSI STANDARDS (repeated violation) Observation: Commercial dishwasher is not installed. Operators have 3 compartment sinks onsite. All equipment to be ANSI certified or approved by the department.
Correction: Unapproved equipment shall be removed from food service.
FOOD PREPARATION SINK Observation: No food prep sink is provided on site.
