Correction: Maintain hot potentially hazardous foods at or above 135°F. Cooked rice was discarded during inspection.

Observation: Cooked rice and a sauce in the walk in cooler were missing a date mark.
Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days. Rice and sauce was disposed of during inspection.
- Critical: FOOD CONTACT SURFACES - WAREWASHING CHEMICAL SANITIZATION EXPOSURE TIME (corrected on site)
Observation: Utensils in mechanical warewashing operation are not exposed to the sodium hypochlorite sanitizer for the required amount of time. Observed sanitizer concentration of 0ppm during first portion of inspection.
Correction: Adjust warewashing operation procedures or equipment to assure utensils are exposed to the chemical sanitizer solution for the required time period. New sanitizer bottle was put on the unit and primed several times. Sanitizer concentration then tested 50ppm during inspection.
- Critical: SEWAGE - BACKFLOW PREVENTION
Observation: A connection exists between the sewage system and back entrance ice bin drain. Currently the system has an air break.
Correction: Design the system in a manner that would preclude a connection between the sewage system and drain. Replace the connection with a proper air gap and ensure the gap is present on all other ice bins. Correct By: 26-Jun-2015

Observation: Delafield two door reach in cooler was observed not holding foods at cold temperatures Cooler temperature was 48-50 F, food temperatures ranged 52 F.
Correction: Provide equipment in good condition to hold food at proper temperature. Food items were moved to the Randall two door reach in cooler during inspection.

Observation: Ice machine, meat slicer, and soda gun nozzles and holders are visibly soiled.
