Observation: Soup prepped for following day use noted without a date mark in the counter cooler. A date was placed on container at the time of inspection.
Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days.

Observation: There is no State of Wisconsin food manager’s certificate posted in the food establishment. Permit was moved out into the serving area, please find a permanent place to display permit and food manager certificate.
Correction: Provide & post an original State of Wisconsin food manager's certificate.
- HANDWASHING SINKS, INSTALLATION - NON-HAND OPERATED
Observation: Handwashing sink is not non-hand operated in the prep area and the bathrooms.
Correction: Employee handwashing sinks shall be non hand operated and if metered shall provide a flow of water without the need for reactiviation for at least 15 seconds.

Observation: Female bathroom is not provided with a covered waste receptacle.
Correction: Provide a covered waste receptacle in female bathroom for use with sanitary napkins.

Observation: Noted during inspection that sanitizer concentration is not being verified. Use test strips to ensure that the ammonia concentration is at 200ppm.
Correction: Provide training to employees on proper use and concentration of sanitizer or repair or adjust warewashing equipment to provide proper sanitizer concentration.
