Observation: Buffet food items using time as a public health control for 4-hours is not properly marked or identified with a discard time. Noted during inspection that new time markers were ordered, old ones placed on buffet at the time of the inspection. Timers are used in the back to pull food items every 20 minutes.
Correction: Food items shall be marked or otherwise identfied with a time that is 4-hours past the point in time when the food was removed from temperature control. Discard food item.
- WAREWASHING - SANITIZING SOLUTION - TESTING DEVICES
Observation: A thermal test strip is not available to verify that the dishwasher is getting the dish contact surface to 160 F.
Correction: Provide a Paper Thermometer type strip to verify hight temperature dishwasher.

- POSTING OF CERTIFICATE
Observation: There is no State of Wisconsin food manager’s certificate posted in the food establishment. There is a current CFM for this establishment, make sure that the certificate is posted in public view next to the Food License.
Correction: Provide & post an original State of Wisconsin food manager's certificate.
- EATING, DRINKING, OR USING TOBACCO (corrected on site)
Observation: Employee was observed eating in the kitchen area. At time of inspection food was moved away from the prep area to a table used for emplyee breaks. Make sure that employee's foods and beverages are kept and consumed away from food prep areas.
Correction: Provide employee break room or designate an area away from food preparation for eating and tobacco use.
- FOOD CONTAMINATION PREVENTED FROM PREMISE - OFF THE FLOOR
Observation: Pizza boxed noted being stored on the floor by the dry storage area.
Correction: Store all food items 6 inches above the floor.
- FOOD CONTACT SURFACES - ENCRUSTED ACCUMULATIONS
Observation: Food−contact surfaces of ice machine noted with mold.
Correction: Clean and remove all encrusted grease deposits and encrusted soil accumulations from food contact surfaces of equipment.

- Critical: PHF/TCS, COLD HOLDING
Observation: Cut lettuce at salad buffet is cold held at [50]°F. It was indicated that salad is switched out and disposed of after 6 hours (11 am - 5 PM).
Correction: Maintain cold potentially hazardous foods at or below 41°F. To maintain 41F a metal container may help and as well as using ice as an additional cooling agent. Or time may be used as a control but a written policy must be developed and documentation must be maintained when using time - when time is used as a control for cold holding food may be held out of temperature control for a maxium of 6 hours if held under 70F and then must be discarded.
- EQUIPMENT AND UTENSILS - GOOD REPAIR
Observation: Dishwash machine temperature gauge does not work during wash cycle.
Correction: Repair equipment to good condition.
