Observation: Chicken (53 F), Ham (48 F), Cheese (43 F) and Tuna (49 F) in the right sandwich make cooler observed above 41 F. The chicken was voluntarily discarded. The lid of the cooler was closed. Discussed always keeping the lid closed when not in use. Also discussed keeping lower portions in the containers.

Observation: Degreaser observed stored above the food prep table.

Observation: Tomato slicer and soda nozzles are visibly soiled. This is a repeat violation from the past 2 years.
Correction: Maintain food contact surfaces in a clean condition.

Observation: The towel dispenser in the men's restroom is not dispensing.
Correction: Provide single-use toweling or other approved devices at handwashing sink to facilitate proper handwashing.

Employee food observed on the second highest shelf of the produce walk-in cooler.
Correction: Provide employee break room or designate an area away from food preparation for eating and tobacco use.
- REMOVING DEAD OR TRAPPED BIRDS, INSECTS, RODENTS AND OTHER PESTS
Observation: Three traps were inspected today and the one by the top door did have a mouse. There is not evidence of an infestation. It appears as though it is an isolated occurrence. Discussed that mice activity generally increases in establishments during the cooler months. The back door was closed and is tight-fitting. Discussed not allowing employees to prop the door open. The pest control company shall be contacted for disposal and resetting the trap. Recommend traps on the exterior of the building. Clean the area near the back door to use as a monitoring tool and to prevent harborage areas. Clean the basement corners and ledges near the stairs. Recommend keeping the interior back door closed as another barrier.
This was also explained last October during a complaint inspection.
Correction: Remove all dead pests frequently enough to prevent decomposition or attraction of other pests.

Observation: The Quat sanitizer noted at 0 PPM. Discovered that the pump was not pulling the sanitizer from the container. Discussed changing the sanitizer container and then retesting the sanitizer concentration.
Correction: Provide training to employees on proper use and concentration of sanitizer or repair or adjust warewashing equipment to provide proper sanitizer concentration. Correct By: 27-Sep-2013
