CERTIFIED FOOD MANAGER – REQUIREMENT Observation: There is no Wisconsin Certified Food Manager for this establishment. This is the third consecutive annual inspection where there is not a certified manager.
Correction: Upon reinspection, Aaron indicated Brad is signed up for a course on April 12th. Notify me at Health Dept when Brad receives the State cerrtification.
NON-FOOD CONTACT SURFACES - CLEAN (repeated violation) Observation: The pans stored under the pizza screens both under the cut table and over the prep cooler are noted with significant accumulation of food debris.
Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment. These pans shall be cleaned daily. Discontinue use of the cardboard trays under the pizza screens.

Critical: FOOD CONTACT SURFACES - FREQUENCY Observation: Food contact surfaces used with potentially hazardous food including the pizza cutters, wing containers and pizza shovel are not cleaned after 4-hours.
Correction: Clean food contact surfaces used with potentially hazardous foods (temperature controlled for safety) a minimum of every 4-hours. Develop a policy indicating when these items are to be cleaned and sanitized.
CERTIFIED FOOD MANAGER – REQUIREMENT (repeated violation) Observation: There is no Wisconsin Certified Food Manager for this establishment. This is the third consecutive annual inspection where there is not a certified manager.
Correction: Provide a Wisconsin Certified Food Manager. Documentation of a certified person or proof that a current manager has signed up for an approved class shall be provided to me within one week. Correct By: 16-Mar-2016
EQUIPMENT AND UTENSILS - GOOD REPAIR (repeated violation) Observation: There is still a gap at the back wall/prep surface juncture of the cut table. This gap accumulates food debris which can easily contaminate equipment, utensils or food.
Correction: Repair or seal this gap as discussed.

Correction: Seal these areas as discussed to provide a cleanable surface where food debris will not accumulate.
