The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.FLOORS ALONG WALLS AND IN ALL CORNERS ESPECIALLY AT BAR AREAS,BASEMENT REQUIRE DETAIL CLEANING.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use.COOKING EQUIPMENTS,EATING UTENSILS IN KITCHEN NOT PROPERLY STORED,INVERTED.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities.LIGHT BULB INSIDE REAR WALKIN-COOLER NOT SHIELDED.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors.REMOVE ALL CLUTTER FROM PREMISES.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.VENTILATION IN TOILET ROOMS IN BASEMENT NOT WORKING.

FOUND NO HEALTH INSPECTION SUMMARY DISPLAYED NOR POSTED FOR THE PUBLICS VIEW DURING THIS INSPECTION.SERIOUS VIOLATION 7-42-010(B).
Food handlers must have hair restraints when working around open food.
Repair water leak at pipe beneath the 3 compartment sink in the kitchen and install backflow prevention device at the mop sink located in the basement closet.
Thermometers are needed,maintained inside all refrigeration units.
Cleaning equipments must be properly stored,remove unnecessary articles from storage areas due to potential pest harborage.
Found Gaskets at the outside rear walk-in-cooler in poor repair,Cutting boards with deep, dark grooves must be sanded/bleached or replaced.
Consumer advisory must be posted.
OBSERVED LIVE FRUIT FLIES IN THE KITCHEN AREA AND THE BAR AREA. APPROXIMATELY 15 LIVE FRUIT FLIES WERE OBSERVED ON THE WALLS, CEILING AND EQUIPMENT IN THESE AREAS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. ALSO, THE FRONT DOUBLE DOORS WERE NOTED ARE NOT RODENT PROOF. THERE IS A 1/4' GAP ALONG THE CENTER OF THE DOORS. SERIOUS VIOLATION 7-38-020.
