One copy of the Food Inspection Report Summary must be displayed and visible to all customers. Last failed summary report was not posted for customers view (found on a cliffboard hanging in the office at rear room), from 5/7/09 visit. Citation issued. Must display summary report at all times, for public view.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Must fix rubber gaskets of small coolers that are in poor repair.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Must clean food stains on exterior of juicer.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Must fix leaky sprayer faucet at 4-comp sink.

All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. THE SOUTH 1 DOOR COOLER DOOR WILL NOT CLOSE, REPAIR SAID DOOR.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. THE FLOORS ARE DIRTY UNDER EQUIPMENT THROUGHOUT THE FACILITY. CLEAN AND MAINTAIN THE FLOORS AND BASEBOARDS SO THEY ARE SMOOTH AND SANITARY FROM CORNER TO CORNER.
The walls and ceilings shall be in good repair and easily cleaned. CLEAN THE DUSTY, SPLATTERED CEILINGS IN THE PREP AREA.
