FAUCET AT 3-COMP SINK IN MAIN COOKING KITCHEN IS PROFUSELY LEAKING & SHOOTING OUT AT THE BASE OF THE KNOB HOLDER, MUST FIX & MAINTAIN. (WRAPPED-UP IN TAPE).
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. FOLLOWING NEED CLEANING: INTERIOR PANEL OF ICE MACHINE, INTERIOR OF FRYERS, EXTERIOR OF GREASE TRAP IN KITCHEN- DUE TO DEBRIS/GREASE/DIRT.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOORS OF WALK-IN COOLER, AROUND GREASE TRAP IN KITCHEN, UNDER FRYERS MUST BE KEPT CLEAN. MUST FIX THE FLOOR TILE GROUTING AT KITCHEN, TO PREVENT PEST HARBORAGE.
The walls and ceilings shall be in good repair and easily cleaned. FILTERS & OTHER ATTACHMENTS TO INTERIOR OF HOOD AT COOKING EQUIPMENT MUST BE KEPT CLEAN.

All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces. OBSERVED BURNED OUT LIGHT BULBS(X) ABOVE POT WASH 3-COMPARTMENT SINK. MUST PROVIDE WORKING LIGHT BULBS.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. POT WASH 3-COMPARTMENT SINK FAUCET LEAKING AT BASE. MUST REPAIR AND MAINTAIN SINK.

CUTTING BOARDS WITH DEEP, DARK GROOVES MUST BE REPLACED.
FOLLOWING NEED CLEANING: LIGHT SHIELDS IN KITCHEN, CONDENSERS & FAN GUARDS OF WALK-IN COOLER, INTERIOR OF CABINETS AT THE BAR.
WALK-IN COOLER FLOORS MUST BE RE-SURFACED (WOODEN SLABS SHOWING), MUST FIX & MAKE SMOOTH & EASILY CLEANABLE.
WALLS & CEILINGS AROUND THE ICE MACHINE & STAIRCASE TO THE BASEMENT MUST BE FREE OF DUST/DEBRIS.
