Molluscan shell stock shall be obtained in containers bearing legible source identification tags or labels. Shellfish shall be obtained from an approved source and kept in their original package until sold. FOUND 2 CONTAINERS OF OYSTERS WITH NO SHELLFISH TAGS. MUST HAVE TAGS WITH PRODUCT AT ALL TIMES. PRODUCT IS BEING DISCARDED AT THIS TIME. TOTAL VALUE $142.00 200 OYSTERS CRITCAL VIOLATION 7-38-005(B2) CITATION ISSUED
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. CUTTING BOARDS IN NEED OF REPLACING. STAINED AND DEEP CUTS. MUST REPLACE AND MAINTAIN.
All food establishments shall be provided with an adequate supply of hot and cold water under pressure properly connected to the city water supply. NO HOT AND COLD RUNNING WATER TO THE TOP OF ICE CREAM MACHINE TO PROPERLY WASH RINSE AND SANITIZE MACHINE. MUST INSTALL AND MAINTAIN. CRITICAL VIOLATION 7-38-030 CITATION ISSUED. All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. ALL COOLERS IN NEED OF THERMOMETERS. MUST INSTALL AND MAINTAIN.

RUBBER GASKETS OF COOLER DOORS AT FRONT COOK'S LINE MUST BE REPLACED. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.
MISSING FLOOR TILE AT STAIRS MUST BE REPLACED. FLOORS AROUND CORNERS & UNDER DRY STORAGE SHELVES OF THE 2nd FLOOR DRY STORAGE MUST BE KEPT CLEAN. FLOOR DRAIN UNDER BIG ICE MACHINE IS SLOW-DRAINING, MUST FIX. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.
CRACKED LIGHT SHIELDS IN DISHROOM & BY THE ICE MACHINE MUST BE REPLACED. Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities.
THE CONDENSATION LINE BEHIND THE SMALL ICE MACHINE MUST EXTEND INSIDE THE FLOOR DRAIN. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.
