Observation: Sveral dented cans of carrots, pear halves, vienna sausage, crunberry sauce, Campbells muchrooms soup stored on shelves. May not use any can that is dented on top, bottom, or side seals. Dented can stored on dry goods shelf. Must discard any can that is dented on top, bottom, or side seals. Removed from shelves all of them- Corrected on site.
- Light intensity
Observation: Missing light bulb in reach in cooler. Must provide adequate light intensity in the area where is food stored inside reach-in refrigerators for easily cleaning and maintaining.
- Food temperature measuring devices are provided and readily accessible
Observation: Missing thermometers in sandwich- reach in cooler, beverage cooler and Pepsi cooler. Temperature was good and bellow 41*F. Must provide thermometer which is easily seen to verify food is at correct temperature and bellow 41*F.
- Storage of clean equipment and utensils
Observation: Paper towels and single use utensils stored on the floor at the back store area. All equipment utensils and single -service items must be stored: In a clean, dry location where they are not exposed to splash, dust, or other contamination and at least (6 inches) above the floor.
- Separation from food, equipment, utensils, linens, and single service
Observation: 1) The bottles of chlorine bleach, Clorox bleach stored above single use utensils, paper towel and plates. 2) Detergent stored next to paper towels. All toxic materials must be separated above food, equipment, utensils, linens and single use articles. Place bellow or separate. Corrected on site.

- Outer openings are protected
Observation: Both outer doors in the back area for trash, deliveries, and emergency exit do not close properly. All exit doors shall be self closing and tight fitting to prevent entrance of pests and rodents. REPAIR AS NEEDED.
- Posting of a valid license
Observation: The current license is posted but not visible due to many other permits were placed in front of the license. Shall post in a conspicuous place for consumers to view.
- Posting inspection reports
Observation: The last inspection report is not posted for consumers to view. SHALL post this inspection at eye level for consumers to view.
- Drying mops
Observation: One mop observed stored improperly for air drying. After use, mops shall be placed in a position that allows them to air-dry without soiling walls, EQUIPMENT, or supplies. CORRECTED ON SITE.
- Food temperature measuring devices are provided and readily accessible
Observation: The reach in cooler does not have a accessible temperature measuring device. Provide a thermometer which is easily seen to verify food is at correct temperatures. Cold hold must be at least 41 degrees or lower. CORRECTED ON SITE.
- Indoor and outdoor surfaces
Observation: 1)The unisex restroom for the public has the lower portion of the wall exposed due to a prior leak. SHALL replace this area to be smooth, cleanable, and nonabsorbent IE: Install the wall.2)There are several tiles that are broken or missing near the managers office. REPLACE THE TILE TO make a smooth, cleanable, and nonabsorbent surface.

- Cleanability of floors, walls, and ceilings
Lower section of wall in public RR is missing around floor. All walls must be easily cleanable and cove base is required at the floor/wall juncture. Make repairs as needed.
- Smoke free air act
Must post state approved "No Smoking" sign at every entrance and exit to be used by public or employees. Window cling given during inspection. Post at entrance.
- Backflow protection

- Drying mops
Mops sitting in water in mop buckets. Mops must be air-dried in between uses. Store mop in a manner that will allow for air drying without soiling walls or equipment.
- Ventilation hood system and filters designed to be cleanable, prevent dripping, and are adequate to prevent grease or condensation from collecting on walls and ceilings
Vent in employee RR is not in working order. All RR must have a working vent. Repair as needed.
