So what is their "real" gender at the time of employment because the majority of society including government forms, at some level require a sexual identity and the employer may be left with no options to make another choice in identifying the individual other than male or female. This is going to get entangled in the courts as employers defend themselves and transgender rights get tested under civil liberty law, win/win for the lawyers. We are also at a point in history where we can no longer rely on natural and obvious physical identities, technology has changed all that through surgical alterations. We've created our own monster, let your imagination run wild at the possibilities for future modifications and identities.

1. Some people out there would like everyone to fit in a little box - male and female, even heterosexual. But the world doesn't work like this, and as far back as history goes, people have not fit into these little boxes. Interesting to note, in ancient Rome it was commonplace - not necessarily openly discussed, but frequently happened - 2. for men to have sex with underage or younger males. Even married men would have sex with younger males. It was accepted, but not really spoken about. .....

A transgender is not born as some 3rd gender. Even artificially, they are not both woman and man, with HRT they are still a man or still a woman until the operation is complete.
I understand and accept those born with both male and female sexual organs as hermaphrodite or even androgynous. But people who want to "transition" into a woman or man and don't identify with their birthed gender need to get the operation, not live their life as a woman with male organs or vice versa.
The biased NYC legislation / LGBT 3rd gender movement is not scientifically or biologically correct, only a psychiatrist worshipping liberal society would call it this. Not even Romans, Mayans, Aboriginese or Navajo, or all the historical periods you can dig up called them a 3rd gender. NOBODY agrees with this except pseudo-psychiatrists. There are 1000s of psychiatrists who could dispute thse claims. Basically pick the medical professional that fits your agenda and run with it.
2. Yes and so do Afghanistan men with boys. Their saying is women and girls are for housework and cooking and boys are for pleasure. Pretty messed up to rape young boys or girls.

It is a mental health issue. What goes on in their head doesn't match their body. It is a type of dysphoria like when a bare bones person looks in the mirror and sees a fat person.
I still think that feeding the fantasy is wrong and only makes it harder in the end. The people that exhibit this behavior usually have a lot going on besides the gender issue, it is a list of symptoms.

No, being transgender in and of itself is not a mental health issue. Anxiety and depression caused by either not being able to accept yourself and/or the way you are treated by others is the issue.
