Perhaps Saipov wasn't vetted properly, or at all, in 2010 when he was allowed to become a permanent resident in America under a lottery visa program? We don't know yet if these murders and injuries of innocent people in New York City could "have been prevented". Your question is unanswerable until more facts are known about this Muslim who committed this terrorist attack in the name of ISIS.
