  • *7.1 Opened, exposed rodent bait on shelf next to pizza boxes located below microwave. Provide a covered, tamper-resistant bait station to prevent possible contamination.
  • R6.4 Observed evidence of mice in rear dry storage area and shelving located below hot holding area. Take steps necessary to control and eliminate rodents.
  • 6.4 Missing wood molding near ceiling above pizza prep cooler. Replace this to provide an easily cleanable surface.

  • *3.4 Packaged ham, turkey, salami, and hot pepper cheese sandwich is dated 9/27. Ready-to-eat foods shall be dated at time of preparation to be discarded within seven days.
  • *7.2 Observed Orange solution stored on retail shelf next to aluminum foil. Properly store poisonous/toxic chemicals separate from food and single-use items to prevent possible contamination.

  • -black residue on edge of ice maker shield
  • -food debris build-up on peppermint patty cappuccino mixing bowl
  • -ice scoop stored on the floor behind ice maker
