• 0820 - Corrected During Inspection Critical Salad dressing and yogurt cold holding at improper temperatures.
  Relocate food to a refrigeration unit that is capable of maintaining food storage at 41F or below. Food was discarded and the unit shall not be used for storage of potentially hazardous food until the unit is repaired to maintain 41F or below.
• 1570 - Three door refrigerator across from the handsink in the kitchen was not maintaining the proper temperature.
  Repair the refrigerator to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the refrigerator, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

• 0450 - Corrected During Inspection Critical Employees observed handling ready-to-eat (RTE) oranges with their bare hands.
  Provide suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment to handle RTE food to prevent contamination from hands.
• 2580 - A condensate drain line was dripping onto food products in the Freezer.
  Repair the condensate drain line to prevent incidental contact with foods. Repair the freezer door heat strip to allow for sealing of the freezer door.

• 0070 - Unauthorized individuals are gaining acces to the ice machine.
  Establish a barrier to food contamination by Informing employees, delivery and maintenance persons and pesticide applicators to comply with this Regulation when entering areas of the food establishment where food and food contact items are exposed. Individuals present in areas of a food establishment where food and food contact items are exposed presents a potential contamination risk. Only authorized employees, trained in food safety should be permitted access to the ice machine.
• 0550 - Corrected During Inspection Ice scoop tray was observed dusty.
  Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 140F and the container.

• 1560 - There is a floor mounted filing cabinet and wooded shelf that is not elevated off the floor or sealed to the floor. A cleaning hazard exists.
  Alter the installation of the equipment to allow easy floor cleaning. Make 1 of the following corrections: 1. Remove the equipment 2. Install casters 3. Elevate the equipment to a minimum of 6 inches off the floor 4. Seal the equipment to the floor
• 2750 - Observed grease drum outside rear exit to be excessively dirty with paint peeling. Large amounts of grease observed on the pad and drum.
  Please replace or repair as needed and empty grease drum more frequently to preclude accumulations of grease on the pad.
• 3170 - Shelvingin the dry storage area was observed missing legs and a can of of food used to support the shelf.
  Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
• 3090 - Ventilation is not sufficient to keep food preparation area free of excessive heat
  Provide mechanical ventilation that is sufficient enough to keep rooms free of excessive heat, steam, condensation, vapors, obnoxious odors, smoke and fumes

• 2930 - Outer opening of the food establishment is not protected against entry of insects and rodents. Screen above 2 compartment sink is not tight fitting, there is a space between the window frame and the screen. This could allow insect entry.
  Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces. Repair screen within 7 days.
• 2700 - The waste storage container (dumpster) for the high school cafeteria has a broken bottom seam and liquid is leaking from the dumpster onto the parking lot surface.
  Replace the waste storage container for the cafeteria within 7 days.
• 0510 - Fruits not washed before being offered for sale or service. Fresh apples are being dispensed from an open bowl on the service line.
  Thoroughly wash raw fruits and vegetables to remove soil and other contaminants before cutting, combining with other ingredients, cooking, serving, or being offered for human consumption in the ready-to-eat form. Dispense apples from a protected container with appropriate utensil or have staff member dispense apples. Correct in 24 hours.
• 0820 - Critical The walk-in freezer and the 3 door refrigerator at the service line were not maintaing required level of refrigeration.
  Relocate food to a refrigeration unit that is capable of maintaining food storage at 41F (45F) or below. Cafeteria manager was moving food to other refrigeration units and had made arrangements to move some items to adjacent facilities. Correcting during inspection. Cold holding units are to be repaired before food items are placed back in them
