The purpose of this visit is a follow-up inspection to determine compliance with replacement of intact, good-repair trash dumpsters and maintenance of the grease barrel outside the facility. One dumpster has missing lids and the second has a lid that does not seal the container and is pushed down into the dumpster. The area around the grease barrel, on the concrete has an accumulation of grease. Within 10 days a follow-up will be conducted to ensure that compliance regarding the dumpster and grease barrel have been met. Thank you.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Please address the fax to my attention and include the name and address of this facility on the fax. Failure to provide a copy of the photo identification card will result in further enforcement action.

    Observation: The waste storage container for the trash dumpster has a missing lid.
    Correction: Storage areas, enclosures, and receptacles for refuse, recyclables, and returnables shall be maintained in good repair. Obtain a dumpster which is in a state of good repair and has intact lids within 10 days.

    Observation: Grease on the concrete area surrounding the grease barrel outside the facility.
    Correction: Clean the grease off the concrete and dispose of either in the grease barrell or in sealed bag and then place the sealed bag in to the trash container within 10 days. As agreed upon. obtain a grease collection pan to place below the grease barrell and relocate the grease barrell to allow room for employees to dump grease cleanly without spilling onto the concrete.

    Observation: The person in charge is not able to provide written procedures for the use of time as a public health control with the following potentially hazardous food item(s): Partially cooked french fries
    Correction: If time only is used as a public health control for a working supply of potentially hazardous foods (time/temperature control for safety food) before cooking or for ready to eat, potentially hazardous foods that is displayed or held for service for immediate consumption, written procedures shall be prepared in advance and maintained in the food establishment and made available to the regulatory authority upon request. The permit holder should be able to describe the process to the Health Department. Manager developed a system for marking discard time for each batch of french fries using white board.

    Observation: he handsink near the three vat sink and the three vat sink faucet are both leaking.
    Correction: A plumbing system shall be maintained in good repair. Email the invoice for the repair of the plumbing for the handsink and the adjacent three vat sink to EHS within 10 business days. I have provided you with my business card which contains my email.

The purpose of this visit is a follow-up to a complaint received by our office on March 28, for grease spilled on the ground and dumpster lids not being maintained closed. The manager has made significant progress in the clean up of the dumpster area for grease. In addition, the manager agreed to place a grease pan below the grease barrell within 10 days. However, the dumpster now has had the lids broken off. In additon to obtaining a grase pan to place below the grease barrell, within 10 days, obtain and maintain dumpsters which are in good repair, to include having a properly closing lid. A follow-up will be conducted on or about April 14, 2014 to determine compliance.

The purpose of this visit is to investigate a complaint of the following alleged violation: "Dumpster's lid was kept open. Grease accumulation is occurred around the dumpster with lid open. " EHS spoke with the manager and the manager agreed to clean the grease barrell, dumpster and area surrounding the dumpsters of grease accumulation. The manager also agreed to maintain the trash dumpsters, grease barrel and dumpster area free of grease accumulation or spills. Ensure that when you clean the dumpster surfaces of grease that you contain the grease to prevent grease from entering the storm drain or contaminating the environment. Place any disposable cleaning materials, such as absorbant cloths, into a trash bag, seal the bag and dispose of into a closed, intact dumpster. It is suggested that you contact your grease handling company to have a grease pan placed below the grease barrell to prevent grease from spilling onto the concrete. In addition, remember to keep the dumpster lid and the grease barrell lid closed after each use.
  • Refuse / Cover Receptacles / Outdoors / Tight-Fitting Lids (corrected on site)
    Observation: Outside refuse container was uncovered.
    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment. Manager agreed that employees will close lid to dumpster and grease barrel each time trash is placed into the dumpster and when grease is dumped into the grease barrell.

    Observation: An accummulation of grease and waste was observed on the concrete area below the dumpster and grease barrell and along the curb leading to the storm drain and into the storm drain
    Correction: A storage area and enclosure for refuse, recyclables, and returnables shall be maintained free of unnecessary items and kept clean. Remind employees to carefully handle the grease waste. It is suggested that you contact your grease handling company to provide you with a pan to be placed below the grease barrell to prevent spills of grease from spilling over onto the parking lot or the dumpster area surfaces.
  • Refuse Receptacle / Clean Frequency
    Observation: An accumulation of grease observed on the sides of grease barrell, the top of the grease barrell and the side of the dumpster directly adjacent to the grease barrell.
    Correction: Remove the grease from the sides and top of the grease barrell and the side of the dumpster. Ensure that the grease contained while conducting the cleanup. Discard any cloths, absorbant materials/ and/or grease removed, and place these into a closed trash bag and discard into a closed dumpster. Soiled receptacles and waste handling units for refuse, recyclables, and returnables shall be cleaned at a frequency necessary to prevent them from developing a buildup of soil, oil or becoming attractants for insects and rodents.
