his was a routine inspection conducted in conjunction with a complaint investigation. A separate report for the complaint was created. One critical violations was observed during this inspection. The manager was advised to review and apply food safety knowledge during operation. The manager was also advised to train food employees on proper and frequent hand washing as well as when to wash their hands to prevent possible food contamination which may lead to foodborne illness and outbreaks.
The facility as a whole was noted to be visibly cleaned.

    Correction: The person in charge failed to state the required temperatures and holding times for the potentially hazardous food items when (1) in refrigerated storage, (2) in hot holding, (3) being cooled and/or (4) being reheated.

  • Plumbing / Maintained in Good Repair
    Observation: Plumbing connections under the 3-vat sink piping are leaking.
    Correction: A plumbing system shall be maintained in good repair. Repair the leak on the piping at the bottom of the 3-vat sink.
  • Refuse / Cover Receptacles / Outdoors / Tight-Fitting Lids (corrected on site)
    Observation: Outside refuse container was uncovered.
    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment.

The purpose of this visit is to investigate a complaint for the following alleged violation: "a tiny, white larvae crawling in the box, not on the hot pizza, on take out order on Sept., 26th, 2014 about 9:30PM. Manager was aware of the complaint. EHS inspected the pizza boxes which were assembled ona dunnage rack near the pizza oven and the pizza boxes in the back, which were wrapped in plastic, unassembled on a dunnage rack in the back area. No larvae observed. Onions in the back area were inspected for larvae and no larvae observed. Drains below the three vat sink inspected and no larvar observed. Ceiling tiles were in good repair with no water or food stains. No larvae or flies observed during to day's visit. The complaint cannot be confirmed at this time and the complaint is closed.

  • Temp Meas Devices for Cold & Hot Hold Equipment, Present (repeated violation)
    Observation: There was no temperature measuring device located in the following cold or hot holding equipment: four door pizza prep cooler
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Refuse Container with Cover/ Women's & Unisex Toilet Room (repeated violation)
    Observation: There is no cover to the feminine napkin refuse container in the unisex bathroom for employees.
    Correction: Provide a covered trash receptacle in the unisex bathroom for employees within 90 days. A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
  • Cleaning Frequency for Physical Facilities
    Observation: Floor drain below the three vat sink is observed with grime around the surface.
    Correction: Clean the floor drain at a frequency so that grime does not accumulate on the surface.

    Correction: All poisonous and toxic materials shall be stored to prevent contamination of food, equipment, utensils, linens, and single service items by separating the poisonous and toxic materials by spacing or partitioning.
  • Critical: Conditions of Use/Presence & Use of Toxics Per Law (corrected on site)
    Observation: The Quaternary ammonium is not being used in accordance with law or the manufacturer's use directions. Tested above 400ppm
    Correction: Poisonous and toxic materials shall be used according to law and per the manufacturer's directions to prevent injury and contamination. Adjusted to 200ppm
