    Observation: Observed the pre-wash spray hose extend below the flood rim level of the sink basin at the garbage disposal. Observed standing water at the garbage disposal and the spray nozzle on the hose stored in it. The device used to retract the hose was observed to be in disrepair. The PIC placed a work order in to repair the hose and removed the hose from the water during the inspection.
    Correction: Install an approved backflow prevention device or alter the length of the hose to provide the necessary separation between the water supply and the flood rim level of the sink basin. The minimum allowable separation distance must be at least 2x the diameter of the water supply inlet and at least 1 inch.

The above observations and the following were discussed with the person in charge:
-Recommend resealing the floors throughout the kitchen. Observed the paint to be worn in several areas.
-Recommend placing thermometers in the hot holding units to ensure temperatures on the external thermometers are accurate.
-Please replace the broken wash gauge on the dishmachine.

Observed droppings on the shelves in the dry storage area, along the sides of the entrance ways, under the 3 compartment sink and on the floors in the dry storage area and custodian closet.

Inspection conducted as a follow up inspection to the May 28, 2013 report.
The above observations and the following were discussed with the person in charge:
-Repair the door sweep under the door at the loading dock. Light was observed under the right door. Sweep was just replaced but needs to be brought down to cover the small gap under the door.
-Fresh mouse droppings observed on the dry storage shelves and under the hot holding station along the front line being used. Concentrate efforts in these areas.
Significant improvements have been made to the facility. Continue further treatment to ensure the issue has been eliminated. Recommend providing covered snap traps once summer school has ended.

    Observation: Observed methods are not being used to control pests. Several apparent mouse droppings observed throughout the kitchen. Observed droppings on the shelves in the dry storage area, under the drainboard of the dishmachine next to the booster heater, under the 3 compartment sink and under the hot holding areas on the front line.
    Correction: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by using methods such as trapping devices or other methods as required.

    Observation: A dead mouse was observed in the control device. Observed a dead mouse in the trap under the shelf in the dry storage area.
    Correction: Remove dead or trapped pests from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.
06/26/2013Follow-up
This follow up was conducted to meet with Mr. Don Upperco with the Spotsylvania County School Board office to discuss and observed the issue with mice in the school, specifically in the cafeteria. Once arriving for the follow up inspection, we were informed that Mr. Upperco was unable to attend due to a meeting this morning. A follow up inspection was conducted in his absence to determine any progress on the issue.
Progress observed in several areas to include the filling in of several holes throughout the kitchen. Sweeps provided under doors. Additional glue traps set.
Areas that need improvement include a large gap between and under the doors where deliveries are being made from the outside of building. Filling in the holes in the ceiling above the fire hydrant next to the handsink by the food prep station. Replacing the missing ceiling tile in the mop room. Repair the significant leak under the rinse sink in the warewashing area. Have certified pest control operators address the issue more aggressively beyond glue trap monitoring.
Please contact the health department by May 31, 2013 to determine the course of action and the projected follow up date.

Observed mouse droppings throughout the kitchen to include the shelves in the dry storage area, under the storage shelves next to the restrooms, under the hot and cold holding stations on the serving lines and under the 3 compartment sink. Pest control methods observed throughout the kitchen only include glue trap monitoring. Ensure pest control methods are being used to eliminate the issue.

The above observations and the following were discussed with the person in charge:
-The 3 door reach in cooler is not in service at this time. The PIC has a work order in place to repair the cooler and parts are on order.
-Please fill in the holes around the pipes over the reach in coolers by the front lines. Please replace the missing ceiling tile in the mop room.
-The final rinse temperature on the dishmachine was measured at 186'F on the temperature gauge and 162'F on the thermometer ran through the dishwasher.
Please provide pest control invoices from the pest control company showing treatments for pest issues.
Staff members are doing an excellent job with continuously cleaning and sanitizing the equipment and surfaces throughout the kitchen!

    Observation: Observed the thermometer on the dishmachine that measures the temperature of the wash water is not calibrated to ensure accuracy. Wash water is measuring over 160'F and the temperature gauge is only reading 120'F.
    Correction: Calibrate food thermometer according to manufacturer's specification as necessary to ensure accuracy

-Recommend covering the potatoes and onions in the dry storage area. Please clean the shelves and floors in the dry storage room.
The quaternary ammonium 146 sanitizer in the 3 compartment sink measured at 200 ppm.
The person in charge was extremely complaint and very knowledgeable regarding food safety, great job!

    Observation: The food-contact surfaces of the following equipment were not observed sanitized. The sanitizer levels in the dishmachine were measured at below 180'F. The PIC has a work order in to replace the heating element. Ensure employees are not using this dishmachine until it can be repaired.
    Correction: After cleaning and rinsing of the food-contact surface, the surface shall be effectively sanitized before coming in contact with food and before use.
