Violation descriptions and comments

Received an anonymous complaint on 1-14-2010 stating that this establishment is serving chicken sandwiches and other sandwich items. Also
received anonymous complaint that establishment is making food elsewhere and bringing it to be served on site.
Spoke with operator and am told that they were "trying new food items". This establishment is licensed as a pre-packaged license. Only frozen
pizzas, and sealed sandwiches purchased from an approved source can be served at the establishment. Absolutely NO "trying new food items" is
allowed. Back area has been partially redone, there were 3 homestyle coolers/freezers, a stove, utensils, plates etc. This back area is not
approved for food service, cease the use of this area. There was some food in the coolers/freezers. Am told its for personal use. Cease and
desist all food service that is beyond what the pre-packed license allows. If this continues legal action may be taken.
Am told the operator would like to possibly upgrade. Left information on upgrading, be sure to contact our department for well/septic,
Waukesha County Zoning and the Town of Oconomowoc Fire and building. Before an upgrade can be approved, approvals must be given from ALL of
the above entities. And, a pre-license inspection will need to be done by our department.

Restaurant Conditional Permit: This Conditional Permit is being issued under the authority of Section 254.64(lp), Wisconsin Statutes, which
reads as follows: The department may condition the initial issuance, renewal, or continued validity of a permit issued under this section on
correction by the permittee of a violation of this chapter, rules promulgated by the Department under this subchapter or ordinances or
regulations adopted under s.254.69(2)(g), within a specified period of time. If the permittee fails to meet the condition within the specified
period of time, the permit is VOID.

This is an upgrade to a category 2 license. The following food items are to be served: Burgers, french dip, grilled ham and cheese, grilled
cheese, soups (bought from a purveyor), chicken breast sandwiches, taco's, hot dogs, steak sandwiches, pork chops, spaghetti and meat balls,
meat loaf, ribs, sunday breakfast (eggs, bacon, toast, french toast, pancakes, biscuits and gravy). Meat will come frozen and then be
reheated in the greaseless auto fryer/cooker.
If any menu items change or are added, the owner must contact our department for approval.
If at any time the conditions of the license are not met or in violation, the license may be revoked.
