Observation: Ice and ice making water is coming into contact with mold on the surfaces of the ice making machine and the bin.
Correction: Empty, disassemble, clean and sanitize the ice machine often enough to keep it free of mold. Correct By: 08-Jul-2015

Observation: Beverages are coming into contact with slime mold in the nozzle of the soda gun. Some of the drip cups also have an accumulation of slime.
Correction: Clean and sanitize these areas. These areas must be maintained free of slime mold. Daily cleaning and sanitizing is recommended. Use a flashlight to see if they are clean. Correct By: 08-Jul-2015
- FOOD CONTAMINATION - FOOD STORAGE PROHIBITED AREAS
Observation: The food ice in the ice bin at the bar has a waste line from the soda gun drip cups going through the ice.
Correction: Remove the waste line from the food ice or designate this ice bin as cooling ice only (not for consumption). Correct By: 08-Jul-2015
- EQUIPMENT AND UTENSILS - BEVERAGE TUBING - SEPARATION
Observation: Beverage tubing is being circulated through the ice bin. The beverage tubing is running past an area that is not clean and its causing chunks of debris to dislodge in the ice
Correction: The beverage tubes may not go through an ice bin that is used for food ice. Use ice from a bin that is only for food ice and does not have tubes going through the ice. Correct By: 08-Jul-2015
- LIGHT BULBS - PROTECTIVE SHIELDING
Observation: Lights located above the cooking area do not have end cap on the plastic shields. Glass fragments may fall out the ends if the bulb is broken.
Correction: Provide shields that have the end caps. Correct By: 21-Jul-2015
- PUBLIC RESTROOMS - GOOD REPAIR AND SUPPLIED WITH TOILET TISSUE
Observation: The urinal does not flush adequately.

- Critical: FOOD CONTACT WITH EQUIPMENT AND UTENSILS
Observation: Bevergaes from the soda guns are coming into contact with slime mold in the gun nozzles and drip cups. Also, the inside surfaces of many of the beer faucets have an accumulation of yeast.
Correction: Clean and sanitize these surfaces often enough to preclude molds and slime. Correct By: 19-Jun-2014
- Critical: FOOD CONTACT WITH EQUIPMENT AND UTENSILS
Observation: Ice making water is coming into contact with moldy surfaces inside of the ice machine.
Correction: Empty, disasseble, clean, and sanitize the ice machine according to the manufacturer's instructions. The interior surfaces must be free of mold. Use a flashlight to make certain the interior surfaces are free of mold. The ice machine must be cleaned and sanitized at the frequency that precludes mold. Correct By: 19-Jun-2014

Observation: The hamburgers in the food prep refrigerator were 45F. Other items in the cooler were found at less than 41F
Correction: Adjust procedures or methods to properly thaw foods and make sure hamburgers do not sit out of refrigeration long enough to be in the danger zone (more than 41F). Correct By: 18-Jun-2014
- THERMOMETERS - FUNCTIONALITY - INTEGRAL OR PERMANENTLY AFFIXED MEASURING DEVICE
Observation: The cooler in the back room and the cooler by the cooking area do not have thermometers installed.
Correction: Equipment used for hot and cold holding shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device. Correct By: 25-Jun-2014
- UTENSILS, EQUIPMENT AND LINENS - STORING PROHIBITIONS - CLEAN DRY LOCATION
Observation: Knives are being stored in the dirty crevasse between the griddle and the table.
Correction: Clean equipment and utensils, laundered linens, single-use and single service articles shall be stored in a clean dry location, not exposed to contamination and off the floor. Correct By: 19-Jun-2014
- PLUMBING - SYSTEM MAINTAINED IN GOOD REPAIR
Observation: The urinal in the men's restroom does not flush when pushing the flush valve.
Correction: The plumbing system shall be maintained in good repair. Repair or replace defective or leaking plumbing. Correct By: 25-Jun-2014
- TOILET FACILITIES - ENCLOSED
Observation: Toilet rooms are not provided with a tight fitting self-closing door.
Correction: Provide a working self- closing door mechanism on bathroom door to provide a tight fitting self closing door. Correct By: 18-Jul-2014
