    Observation: There are ready-to-eat dressings and sauces stored under raw shelled eggs in the walk-in cooler.
    Correction: Reorganize storage so that ready-to-eat foods are stored above raw animal food and raw vegetables. Discard ready-to-eat foods that show evidence of contamination. Eggs were moved to the bottom shelf during the inspection.
  • Critical: PHF/TCS, HOT HOLDING (corrected on site)
    Observation: Pork in pot on oven was observed hot held at 125.6 F.
    Correction: Maintain hot potentially hazardous foods at or above 135°F. Remaining pork was discarded during inspection.
  • Critical: TOXIC SUBSTANCES - CONDITIONS OF USE - USED (corrected on site)
    Observation: Sanitizer in front bar sink for dish cloths was not being used according to manufacturer’s use directions. Sanitizer level was observed at 200 +.
    Correction: Change procedures and provide training to employees on proper use of toxic chemical following manufacturer's directions for use. Sanitizer was diluted to appropriate level during the inspection.

    Observation: Working containers of sanitizer was observed not labeled with contents in the back storage room.
    Correction: Label working containers. Common name was written on container during inspection.

    Observation: Drinking cup without cover was observed in food prep and kitchen areas.
    Correction: Provide approved beverage container in food preparation area. Drinks were emptied during the inspection.

    Observation: Facility foods are subject to potential contamination by variety of employee foods and drinks (soda, canned pickles, etc) in the walk-in cooler.

    Observation: Waste receptacles located outside facility were observed to not be covered.

    Observation: There is an open hole in the back storage room ceiling above the electric water heater.

    Observation: There is a hole in the bottom and a gap in top of the back screen door of the kitchen which could allow the entrance of insects or rodents.

    Observation: No handwashing signage provided at kitchen and men's restroom handwashing sinks.
    Correction: Provide handwashing signage at all handwashing sinks used by food employees. Sign was moved to the kitchen handwash sink and additional signage was given to facility during inspection. Correct By: 25-Aug-2015

    Observation: Employees are unaware of the illness reporting requirements to the person in charge.
    Correction: Provide training to all employees on the signs and symptoms of foodborne illness and explain the requiremnts for reporting to the person in charge. Inspector will email information regarding employee health and hygiene and an employee reporting agreement. Correct By: 19-May-2014
  • Critical: PREVENTING CONTAMINATION FROM HANDS - NO BARE HAND CONTACT WITH RTE-FOODS
    Observation: Employee observed handling bread, lettuce, cheese and buns with bare hands.
    Correction: (REPEAT) Employees shall use single-use gloves, utensils, bakery papers or other approved means when handling ready-to-eat foods. Educate all employees about using utensils, gloves or other means to avoid bare hand contact. Correct By: 14-May-2014

    Observation: There are ready-to-eat chopped onions, deli sliced chicken breast and cheese stored under raw steak and raw shell eggs in the walk-in cooler.
    Correction: Reorganize storage so that ready-to-eat foods are stored above raw animal food and raw vegetables. Discard ready-to-eat foods that show evidence of contamination. During the inspection the walk-in cooler was reorganized. Correct By: 12-May-2014

    Observation: Beans and deli sliced chicken breast in the walk-in cooler are not date marked.
    Correction: Datemark all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, to effectively identify when a food item has exceeded 7-days. Beans were discarded during the inspection. Correct By: 13-May-2014

    Observation: Bottles of Windex, Febreeze, and other cleaning chemicals are stored over linens and clean dishes in the dry storage area.
    Correction: Rearrange area so that toxic materials are stored below or away from food, equipment and single service items. Correct By: 14-May-2014
  • Critical: TOXIC SUBSTANCES - MEDICINE - REFRIGERATED STORAGE
    Observation: There is a bottle of employee medication that is stored in the Turbo Air reach-in cooler behind the bar. It is not stored in a covered, leak proof container and is stored above clean glasses and sauces.
    Correction: Store employee medicine in a covered leak proof container. Correct By: 13-May-2014

    Observation: The consumer advisory on menu is missing asterisks next to each food item or food grouping that could be served in a raw or undercooked condition.
    Correction: Provide an asterisk next to the consumer advisory and next to animal (or groupings of foods) that could be served in a raw or undercooked condition. For example: if all hamburgers could be served in an undercooked or raw state, then either each burger on the menu or the heading that says "burgers" should also have an asterisk. Do not asterisk the children's menu-children may not order undercooked or raw animal foods from the children's menu. Correct By: 12-Nov-2014

    Observation: The soda gun holder on the bar nearest the exit has accumulations of white slime and is in need of cleaning.
    Correction: Maintain food contact surfaces in a clean condition. Clean soda guns and holders on a regular basis. Correct By: 14-May-2014
  • OPEN DRINK CONTAINER
    Observation: An employee drinking cup without a cover is stored on the shelf next to the microwave and above the loaves of white bread. If the cup is knocked over, it has the potential to contaminate all the loaves of bread.
    Correction: Provide approved beverage container in food preparation area. Correct By: 19-May-2014
  • IN-USE UTENSILS, BETWEEN USE STORAGE (corrected on site)
    Observation: Ice scoop in the ice bin nearest the front exit of the bar was improperly stored with the handle laying in the ice.
    Correction: Store in-use utensils in the food item with handle extended, on a clean surface, running dipper well or in a container of water greater than 135°F. During inpsection, the ice scoop was moved and stored with the handle sticking out of the ice to prevent contamination. Correct By: 12-May-2014
  • UTENSILS, EQUIPMENT AND LINENS - STORING PROHIBITIONS - CLEAN DRY LOCATION
    Observation: Single-use napkins and cups are stored in boxes on the floor in the hallway leading to the men's bathroom.
    Correction: Clean equipment and utensils, laundered linens, single-use and single service articles shall be stored in a clean dry location, not exposed to contamination and off the floor. Store all single-use utensils off the floor. Correct By: 19-May-2014
  • PLUMBING - SERVICE SINK
    Observation: There is no service sink provided in food establishment for dumping dirty mop water.
    Correction: Provide at least 1 service sink or curbed cleaning facility for dumping dirty mop water. Correct By: 12-May-2015
  • HANDWASHING SIGNAGE
    Observation: There is no handwashing signage posted at the handwashing sink in the kitchen and at the handwashing sink in the men's bathroom.
    Correction: Provide handwashing signage at all handwashing sinks used by food employees. Relocate the handwashing sink posted above the food preparation sink to the kitchen handwashing sink. Correct By: 19-May-2014
