Observation: The pepper mix and salsa in the walk in cooler are not date marked. The sliced ham in the True 3 glass cooler has the incorrect date, and the container of chocolate chips is labelled as "onions".
Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days.
- Critical: EQUIPMENT AND UTENSILS - MATERIAL FOR CONSTRUCTION AND REPAIR - UNSAFE
Observation: The chips are stored in a garbage can. The rice in the walk in cooler is stored in plastic grocery bags.
Correction: Discard unsafe equipment and utensils.
- Critical: TOXIC SUBSTANCES - SEPARATION
Observation: There is a spray bottle of sanitizer stored above the condiments and silverware in the server station. Apple juice was stored under the sink in the bar with the dish chemicals.
Correction: Rearrange area so that toxic materials are stored away from food, equipment and single service items by spacing or partitioning.

Observation: The consumer advisory is not on all of the menus, nor is the asterisk placed next to the foods which require a consumer advisory. .
Correction: Provide a consumer advisory for animal foods served in a raw or undercooked condition.
- THERMOMETERS - AMBIENT AIR AND WATER - ACCURACY
Observation: Thermometer in the Victory prep cooler measuring ambient air is not accurate. The thermometer in the Beverage Air salsa cooler is broken.
Correction: Calibrate or replace thermometer.
- FOOD CONTACT SURFACES - SOILED
Observation: The can opener has rust and food debris on the blade.
Correction: Maintain food contact surfaces in a clean condition.
- PACKAGED AND UNPACKAGED FOOD - COVERED CONTAINERS OR WRAPPING
Observation: The onions and peppers in the walk in cooler on the top shelf are not covered. The frozen fish in the small chest freezer are not covered.
Correction: All foods that are not part of a cooling process shall be covered at all times to prevent cross-contamination. If cross-contamination is evident, discard suspected foods.
- IN-USE UTENSILS, BETWEEN USE STORAGE
Observation: The ice scoops in the bar and server beverage station are not stored with the handle extended.
Correction: Store in-use utensils in the food item with handle extended, on a clean surface, running dipper well or in a container of water greater than 135°F.
- WIPING CLOTHS - USE LIMITATION - CLOTHS FOR WIPING COUNTERS STORED IN SANITIZER
Observation: Wiping cloth used for wiping counters stored on the counter.
Correction: Cloths used for wiping counters shall be stored in a sanitizing solution.

Observation: There is raw meet stored on the floor in the walk in cooler. There are onions stored on the floor in the dry storage area.
Correction: Store all food items 6 inches above the floor.

Observation: There are gaps between the baffles in the exhaust ventilation hood system.
Correction: Provide, repair, or replace the exhaust ventilation hood system so that grease or condensation is prevented from draining or dripping onto food, equipment, utensils, linens, single service or single use articles.
- EQUIPMENT AND UTENSILS - GOOD REPAIR (repeated violation)
Observation: There is duct tape on the cover of the large chest freezer. The True 3 door cooler has been removed.
Correction: Repair equipment to good condition or remove from premise.

Observation: The interior and exterior surfaces of the prep cooler are soiled.
Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.

Observation: The underside of the pass-thru window is not finished and has exposed raw plywood. The outer edges of the pass-thru window have gaps that need to be finished.
Correction: Repair area or replace with approved materials. Walls, floors and ceilings shall be smooth, durable and easily cleanable. If located in areas subject to moisture they shall also be non-absorbent.

There is duct tape on the cover of the large chest freezer. There are two holes on the front lid of the small chest freezer with insulation exposed.
Correction: Repair equipment to good condition or remove from premise.

Observation: The interior and exterior surfaces of the prep cooler are soiled. The bottom interior surface of the 3 door freezer is soiled.
Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.
- SURFACE CHARACTERISTICS - INDOOR AREAS
Observation: There are ceiling tiles in the kitchen that have been moved causing gaps in the ceiling. The edge of the pass-thru window is not finished and has exposed raw plywood. The sides of the bar have raw plywood. The backsplash of the bar is raw drywall.
Correction: Repair area or replace with approved materials. Walls, floors and ceilings shall be smooth, durable and easily cleanable. If located in areas subject to moisture they shall also be non-absorbent.
- LIGHT BULBS - PROTECTIVE SHIELDING
Observation: The ceiling lights in the dish washing area do not have the end caps on the tubes. The lights in the prep area of the kitchen are not covered.
Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs.
