Observation: The walk-in cooler shelving, kitchen preparation cooler interior and shelving and interior of the microwave oven are not clean. There is mold/mildew buildup and food residue on the interior of the units and on the shelving. Note: The ice machines are empty and scheduled for cleaning/servicing.
Correction: Thoroughly and safely clean and remove mold/mildew, dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment. Contact the services of a refrigeration professional for assistance in cleaning the fan guards and behind the fan guards.
- WATER FROM APPROVED SOURCE - SAMPLE REPORT
Observation: A recent water sample report for the establishment was not available. Provide for change of operator.
Correction: Retain a recent water sample report onsite for review by inspector.

Observation: The walk-in cooler walls and ceiling have a mildew/mold buildup. Some cleaning has been done.
Correction: Finish cleaning the walk-in cooler walls and ceiling. Thoroughly remove all build-up. Clean the physical facilities at a frequency necessary to keep them clean.
- PUBLIC RESTROOMS - GOOD REPAIR AND SUPPLIED WITH TOILET TISSUE
Observation: The upstairs men's restroom is very odorous. The women's room ceiling drywall tape is coming apart.
Correction: Thoroughly clean and repair the toilet rooms.
