- HANDWASHING CLEANSER AVAILABILITY (corrected on site)
Observation: Soap not available at food employee handwashing sink in the bar.
Correction: Provide hand soap at handwashing sink to facilitate proper handwashing.
- CERTIFIED FOOD MANAGER – REQUIREMENT
Observation: There is no Wisconsin Certified Food Manager for this establishment. Susan has taken the course held at Machuts but has not obtained a certificate through Wisconsin.
Correction: Provide a Wisconsin Certified Food Manager and post the state certificate in the food establishment.
- EQUIPMENT AND UTENSILS - GOOD REPAIR
Observation: The metal step to the walk in cooler is loose and is not maintained in good repair.
Correction: Repair equipment to good condition or remove from premise.
- NON-FOOD CONTACT SURFACES - CLEAN (repeated violation)
Observation: Non-food contact surfaces of the ventilation hood, kitchen cabinets, and the upstairs kitchen floor are soiled with grease, food debris, and dust.
Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.
- NON-FOOD CONTACT SURFACES - CLEANING FREQUENCY
Observation: The ventilation hood is not cleaned on a frequent basis.
Correction: Clean all equipment on a routine basis to prevent accumulation of soil residues.
- PUBLIC RESTROOMS - WASTE RECEPTACLES
Observation: The garbage can lid in the upstairs bathroom is covered with duct tape and is not easily cleanable.
Correction: Provide covered waste receptacles that are easily cleanable in restrooms used by women .

Person in charge has not informed or required food employees to report signs, symptoms, or diagnosis of foodborne illness.

Observation: The basement kitchen cabinets and equipment need some additional cleaning to remove grease accumulations. Defrost the small freezer.
Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.

Observation: The small refrigerator needs to be defrosted.
The soda gun cups need to be cleaned.
The downstairs kitchen equipment and cabinets needs some additional cleaning.
