Of course, we're probably talking about cultural Southernness here. That title probably goes to Louisville, KY or Salisbury, MD. (Most of Maryland doesn't really have a distinctive culture, whether Northern, Southern, or other - same thing with NoVA.)
Looks like the hats old ladies wear at church. The old ladies that sit in the front, blocking everyone's view because of those hats. Seriously, those hats aren't really a MD thing; moreso a Preakness and old church lady thing.

I just find it to be an interesting southern twist to the area. Just like Old Ebbitt Grill, mint juleps and the popularity of seersucker suits during the summer in DC.

Also, keep in mind that Southern MD, and the Eastern Shore have a different history than the Baltimore area. Baltimore was a very "white" city before the Great Migration. Before that, it was very Irish and to a lesser extent, Italian. During slavery, there were far more Free Blacks than enslaved.

Also, northern states, Like NY, only ended slavers 36 years before everyone else. Before then 25% of NYC's population were slaves.
