"Can't afford to pay" is relative. It depends on how much one's bills are, so that's irrelevant. Either ALL US 1040 filers pay federal income tax, or NONE do.

I think those who are able should chip in at a reasonable level. Reasonable people can debate about that reasonable level.

We have the right to get treated by a qualified medical doctor, and it should be regardless of the size of my wallet.

Your scenario is ridiculous. If you aren't a doctor, I don't want you touching me in that scenario
