I will say that when you're 23 years old, not being called hot and not being called cute by any members of the opposite sex when your friends are, seems like a reason to commit suicide, but when you're 50? Meh

Because it's two different things.
I'd venture to guess there's a decent number of guys who have made it to age 50 without being called hot or cute. And there's MANY more who have only heard it a handful of times.
I can actually remember the handful of specific women in my life who have called me cute ... over 40 some years.
It's happened that seldomly.
As for how to find a woman to date you if she doesn't find you attractive off the bat, I'd have some pretty good advice for that, as that's the life I've lived.
It is a real thing. For males who never manage to form an intimate partnership, they start getting overwhelmed by the process. They haven't had much success asking women out, then they get real self conscious about there lack of sexual experience, that stresses them out and they start having more anxiety about meeting women. The sex surrogate mostly teaches them about how to get more comfortable with there own sexuality and how to have more successful and appropriate ways of sharing with someone new.
With women, there is less of a taboo about a lack of sexual experience even into your 40's and beyond. So I suspect that is why you don't see them seeking out this type of therapy the way men do.

Not picking on you but I disagree with this. I actually think I look worse smiling because it is just not natural for myself. A lot of people I know can say the same. Plus I find being told to smile a little insulting. I think a lot of women feel the same way about this sentiment.
In Eastern Europe and possibly central Europe as well, generally you only smile at people that you already know and like as an actual current friend. If you are smiling at people you don't know you are assumed to be insincere and kind of fake. So the notion of smiling to be friendly has a completely different connotation.
