2-401.11 Store employee food and personal drinks in a designated area below and away from food, utensils, equipment, linens, and unwrapped single service/single use articles. /Employee food and drink was stored on prep table above dry bulk products and beside deli slicer. These items were moved to a designated location, and violation was COS.
ADPH 420-3-22.02(3)(a) Provide at least one designated person with certification from an accredited program within the past 5 years, or verification of attendance of an accredited program within the past 12 months.
3-501.16 Maintain potentially hazardous food at 41 F or below. /Cooked chicken fingers on the counter at a temperature of 69-70F. Chicken was discarded, and violation was COS.
3-501.17 Properly date mark all potentially hazardous food prepared in-house when held for more than 24 hours. /Containers of almond chicken and chicken wings in WI cooler were not date marked. These foods were discarded, and violation was COS.

4-601.11 Clean interior of ice machine as often as necessary to prevent accumulation of soil and mold.
4-601.11 Clean storage shelves and exteriors of equipment throughout the establishment. Clean top of stove and inside oven.
4-201.11 Provide durable stoppers for the warewashing sink compartments that are designed and constructed to retain their characteristic qualities under normal use conditions. /Stoppers were broken and employees were using washing cloths for wash and sanitizing compartments of warewashing sink.
5-205.15 Repair condensate leak in WI cooler.
