Operator to contact health dept. prior to new proposed kitchen remodeling. Facility is not presently preparing food, just pre-packaged snacks. |

Will conduct a re-inspection to verify compliance. RE: floor tiles in bar area. Owner to remove existing tiles in bar area and replace with smooth, easily cleanable floor surface- extension given to replace floor in bar area (TTOC date of March 7th, 2013 has been established- will conduct re-inspection after that date to verify compliance). |
