Item 15A- wall in womens restroom has been patched and painted; ceiling tiles in mens room has been replaced; floor tiles have been removed from behind bar. Operator has rented a grinder/sander to smooth out concrete floor this weekend. Floor will need be sealed following weekend.

Regarding floor tiles missing in bar area, operator has made repairs to missing tiles. Existing mop sink was re-located to rear storage room. Operator to contact health dept. prior to new proposed kitchen remodeling. Facility is not presently preparing food, just pre-packaged snacks.

 02/07/2013InspectionRegarding sanitizer test kit and Men's rest room door- a time table of compliance date of February 20th, 2013 has been established to correct these violations. Will conduct a re-inspection to verify compliance. RE: floor tiles in bar area. Owner to remove existing tiles in bar area and replace with smooth, easily cleanable floor surface- extension given to replace floor in bar area (TTOC date of March 7th, 2013 has been established- will conduct re-inspection after that date to verify compliance).
