Observation: Sanifect Plus 2 Fresh 'n Clean (EPA # 47371-131-7546), used in the sanitizer spray bottle, is not an Wisconsin approved sanitizer.
Correction: Sanitizer shall meet the requirements under 40 CFR 180.940.

Observation: Buckets of pickles are stored on the walk-in cooler floor.

Observation: The exterior and interior of the four drawer cabinet, under the flat top grill, is soiled with grease and food debris.
Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.

Observation: Clean plates, covered in plastic wrap and stored on a bottom shelf, are subject to contamination from the fryers.
Correction: Clean equipment and utensils, laundered linens, single-use and single service articles shall be stored in a clean dry location, not exposed to contamination and off the floor.

Correction: Clean the physical facilities at a frequency necessary to keep them clean.

Observation: There is no current food manager’s certification posted in the food establishment.
Correction: Provide & post state food manager certification. Matt will call DHS for a new copy of his current certificate.

Observation: Foods are stored below an unprotected sewer line in the basement.
Correction: Remove food items from this area to an area not subject to contamination, or install a gutter below the sewer line that drains away from the food and equipment storage areas.

Observation: The NSF freezer in the kitchen has a broken handle and is repaired with duct tape, not cleanable.
Correction: Repair equipment to good condition. Repair so it is smooth, durable and easily cleanable.
