My insurance is $78 a month. That comes with a $2500 deductible per occurance. This means that if I step into the street and get hit by a bus it will cost me $2500 dollars. All other expenses deemed necessary by the insurance company will be paid for. If I recover and get run over by a gangster in a Mercedes fleeing the police outside the hospital that is another occurance and another $2500 out of my pocket.
I think my daughters household pays about $170 a month for 2 adults and 2 kids. That's through an employer.
My other daughter is self employed. Insurance for her and her son would cost her about $800 a month if she had to pay it herself. Fortunately her husband gets it through work. She has some small business liability type insurance but I have no idea what she pays for it. Plans can vary widely.
Some other costs can be ridiculous. A ride in an ambulance is said to be $2500. Birth of a child can be between $10000 and $30000. I remember I had a disagreement with a cobblestone street way back in 1998 on my bike. I forgot about the old trolley tracks and basically landed face first on the street. I staggered to a nearby hospital emergency room where they gave me 9 stitches above my eye. A $500 deductible for me and $1100 billed in total.

In Russia, many medical procedures are completely free for patients. A couple of months ago, my little child (5 months) fell from a couch to floor. We called for help soon (ambulance), doctor arrived in 4 minutes (of course, I understand, this is the Moscow region and in other regions everything is much worse). Doctor not found something wrong, but still the ambulance took my spouse and child to hospital’s admission point, traumatologist on duty examined the child (waiting line was about 15-20 minutes) gave recommendations and let them go home. It was completely free, this is paid from budget (from taxes).
Of course, queues and a large number of patients make free medicine inconvenient, so many choose private clinics (if they can afford it).
