Wow. No. In Illinois, a voter registration application only requires one to acknowledge that any U.S. citizenship claims are valid. Easily abused.
No in IL it requires:
A. To Register Online
(1) the full Illinois driver’s license or State identification card number; the last 4 digits of your social security number; and
the date the Illinois driver’s license or State identification card was issued.
B. To Register in Person
(1)Two forms of identification with at least one showing your current residential address.
C. To Register by mail
(1) driver's license number or State ID card number;
(2) and if you don't have those you can use the last 4 digits of SSN plus one of the bellow:
a. Copy of a current utility bill, bank statement, paycheck, government check, or other government document that shows your name and address.
b. copy of a current and valid photo identification
c. photo identification issued by a college or university accompanied by either a copy of the applicant's contract or lease for a residence or any postmarked mail delivered to the applicant at his or her current residential address.
What that check box does is provide a legal means to charge someone if they abuse the application.

One could get anyone's electric bill out of their box with the name and address, check the i dont have any of these identification numbers and get a registration card showing the voting precinct. If that person doesn't vote you can take their vote, even better if they are deceased since you dont have to prove who you are at the polls.
