There were multiple witnesses and the woman whom he spoke to confirmed the account. That is sufficient for confirmation.
Problem is he doesn't know when to leave something alone. I fully believe he called and offered what were meant to be heartfelt comments. If he tripped up on the name or didn't say it correctly or if the grieving widow mistook his meaning he should have left it alone.

I agree with this. I can't, even in my most disgust filled thoughts of Trump conceive of him deliberately offending a soldier's widow. But what would any of us do in this circumstance where we hurt rather than comforted? Call the person a liar? Who does that?

Trump later referred to this widow as " the woman". He obviously did not know the fallen soldier's name, or he would have called her Mrs Johnson.
Senility? I doubt it. Trump has been this was his entire adult life.
Calls like this are scripted. Does not take much time/ energy to put fallen soldier's name and an appropriate level of personalization into the script. Does not take much time to rehearse the call. No idea if instead, Trump chose to wing it, as he has been prone to do.
He previously insulted a Gold Star family during the campaign. Who does this? He insulted a POW. He like soldiers who were not captured and all that from a man diagnosed with a heel spur and deferrment who instead of serving, played golf and tennis, despite the heel spur.
Melania makes excuses for her husband with the " he punches back, twice as hard" thing, to absolve him of responsibility for his outrageous behaviors. He has surrounded himself with people who make excuses for him, his entire life.

Make up what you want about his intentions, he tried to console and/or comfort her and it blew up into a national story because he allegedly said it wrong and a congresswoman was there to gleefully report on his misstep.
